Can Usher Syndrome Type III Be Detected Genetically?


Usher syndrome Type III is the rarest form of genetic disorder. As a genetic disease, there are tests available to diagnose this illness, including prenatal tests.
What Is Usher Syndrome?
Usher syndrome is a genetic disease that affects hearing and vision. It also goes by the names Graefe-Usher syndrome, Deafness-retinitis pigmentosa syndrome, and dystrophia retinae pigmentosa-dysostosis syndrome. It is a progressive disease so that sufferers at first experience mild symptoms such as difficulty being able to see at night. As the disease worsens, individuals eventually develop blind spots and tunnel vision. While those who have it retain some vision, most patients become completely deaf.
Types of Usher Syndrome
There are three main types of Usher. Type I affects individuals throughout their whole lives. Those with Type I are born deaf or completely lose their hearing within their first year of life and they start losing their vision in childhood. Type II patients are also born deaf or experience complete hearing loss during the first year. However, the onset of vision loss starts later, usually around adolescence. Those with Type III experience symptoms later in life. Unlike Type I or Type II, Type III infants are able to hear and do not experience hearing loss until they approach adolescence. Vision loss begins to occur around the same time.
Rarity of Type III
Overall, this is a rare disease. On average, it occurs in just 4 out of 100,000 people. However, because it is a genetic disease, it occurs with a higher rate in certain populations such as eastern and central European Jewish ethnicity. Even among patients, Type III is rare. The sole exception lies within the Finnish population where 40 percent of patients have Type III.
Usher Syndrome Type III Genes
Several genes produce Usher Syndrome, but Clarin One, or CLRN1 is the only gene identified as being responsible for the Type III disease. This gene plays a crucial role in the development of vision, hearing, and balance. The disease requires at least two mutations; one of the CLRN1 gene and another gene yet to be identified.
Genetic Tests
Since the CLRN1 gene mutation is a hallmark of Usher syndrome Type III, genetic testing is available. Carrier tests can determine if someone has the potential to pass along Usher syndrome to their offspring. Prenatal tests can determine if a child will be born with the disease.
Sometimes it may seem tempting to avoid these tests and hope for the best, but doing so puts your offspring at more risk. If you know that a genetic disease is a high possibility, you can prepare ahead of time, and know what to do if it happens. You could have doctors ready to help you, and information on hand. Burying your head in the sand is rarely the right thing to do in any situation, and when it comes to genetic testing for potential diseases one should definitely consider the many up-sides.

Genetic Testing For Tay Sachs Disease


Tay Sachs Disease is a condition in which a baby's neurological system begins to deteriorate about three to six months after birth. It is a genetic condition, and there is no known cure for the disease. It is not detectable before or at birth except with genetic testing. Unfortunately, children are often not diagnosed until the deterioration has begun. It is fatal, so all too often, parents watch their children die from an untreatable condition. Most children who die of this condition will do so at age four or five. Very rarely, people have developed this condition after becoming adults.
Genetic testing is available for Tay Sachs Disease. Although this disease is inherited, doctors call it a recessively inherited disease. This means that both parents must carry the gene for the disorder for the child to inherit it. If both parents do carry the gene, each child has a 25 percent chance of developing the disease. Although these aren't great odds, this does mean that parents who have had a child with the disorder have a three in four chance of conceiving a child who does not have the disease. This can give hope to some parents who are desperate to conceive their own child despite the potential genetic risks.
Testing can be performed before the mother gives birth, or after the child is born. Some parents choose to undergo the testing before birth so they can be prepared for this condition. Other parents may choose to terminate the pregnancy based on the results of the testing. If the testing is performed after birth, it is generally done as a confirmation, to verify what the doctors have already suspected.
Some parents, though, would rather not know, and choose to have the child even after they know their children are at risk for inheriting the condition. There is no wrong answer. Each parent has to choose for himself or herself whether the pain of giving birth to a child who will certainly die from an untreatable condition is greater or less than the pain of never having a child at all.
Children with Tay Sachs Disease often develop blindness, have seizures, lose muscle control, and become less alert. No medications are available to prevent the problem from occurring. All doctors can do is to help to decrease the frequency of seizures, and offer other medications to lessen the severity of symptoms. In some cases, though, experimental therapies like stem-cell transplants and cord blood transfusions have greatly reduced or eliminated the symptoms of this condition. This has been only partially successful, though, and only if the child is diagnosed and treated immediately or shortly after birth.
Two groups of people are at very high risk for the disease in comparison with the rest of the world's population. The Ashkenazi Jewish population has one chance in 27 of developing the disorder. In comparison, French Canadians have about one chance in 73 of inheriting this condition. The rest of the people born throughout the world have, on average, a one in three hundred chance of inheriting this disorder.

What Is Carrier Screening?


Carrier screening is a method used to provide information about the probability of possible genetic disorders. It helps determine if a parent is a carrier for disorder and if he or she could possibly pass a specific disorder onto offspring. Carriers of genetic disorders have one gene that is considered normal and one that is mutated. Carriers of genetic disorders are typically very healthy individuals who never see any effect of the mutation. However, the mutation can be passed on to a child. Being a carrier depends on one's ethnicity and genetic makeup. This means some ethnic groups have a higher rate of genetic disorders in comparison to others.
When Should Carrier Screening Be Done?
Carrier screening should take place if there is a history of genetic disease in a family. Many couples planning to have children will undergo carrier screening in order to determine how risky the pregnancy could potentially be. Certain ethnic groups choose to be screened, as well, in order to make sure genetic diseases such will not be passed down.
How Are the Results of Carrier Testing Beneficial?
Carrier testing results are deemed positive or negative. Negative screenings indicate no mutations were found. This means the person being screened has a low probability of being a carrier but it does not eliminate the chance completely. Positive screenings typically mean a mutation is present. When this happens, it is suggested that one's partner undergo genetic screening, as well. If a couple both are determined to be carriers, there is an extremely high chance that their child will be born with a disease such as Tay Sachs disease or muclolipidosis. When a person finds out if he or she is a carrier of a genetic disease, they should notify family members so they can determine if they would like to be screened too.
What Are Some Genetic Diseases Detected by Carrier Screening?
Genetic diseases such as Tay Sachs disease can be helped through carrier screening even though there is no cure for this specific disease. Parents can become more informed of the background of the disorder and understand what precautions should be taken during the early infant months. It is a disease caused by a missing enzyme. This enzyme is commonly known as HEXA and it causes the brain and nervous system to deteriorate at a very fast rate. Those affected by this disorder appear to be in good health during the first few months of life. However, at ages 3-6 months, developmental skills start to disappear. Children with this disease usually have bouts with blindness, seizures, and loss of strength. Their lifespan is short and most die around the age of six. Children affected by this genetic disorder have to be kept comfortable as there is no particular cure.
Tay Sachs disease is most prevalent in the Ashkenazi Jewish population. This ethnic group has a 1 in every 3,600 chance of being affected, whereas most other groups only have a 1 in 360,000 chance. The Ashkenazi Jewish population also has a high carrier frequency of 1 in 27. Many other genetic disorders such as muclolipidosis, cystic fibrosis, and Usher Syndrome Type III can all be detected by carrier screening. Is There Such a Thing As a Perfect Diet?


Is there such a thing as the perfect diet? If so what would it look like? For years all of us have been bombarded by all kinds of diets. Low carb, low fat, high protein, and on and on. But with all the research isn't there one perfect diet that works for everyone?
Well the short answer is maybe. First let's look at what I believe would the qualifications for a perfect diet.
  1. Is made up of wholefoods.
  2. Be economical.
  3. Consists of foods readily available in your local grocery store.
  4. Consists of many of the foods/meals we consider traditional or comfort foods.
  5. Be healthy.
  6. Help you to maintain ideal weight.
  7. Become a lifestyle not a diet.
  8. Consist of foods & meals the whole family will love.
Okay that's a pretty tall order, but doable. After all shouldn't this perfect diet be one that everyone could enjoy and not some diet that you tolerate for a few weeks, lose some weight and then go right back to the burgers and fries. Well stay tune and I'll explore each of these qualifications and see if we can come up with a perfect diet.
First of all let us look at whole foods. By whole foods I mean foods that have one ingredient. Like salmon. The only ingredient should be salmon! Or chicken, potatoes, spinach; okay you get the idea.Select foods that are onprocessed and do not come with a list of ingredients longed than one or two items and certainly not with ingredients you can't pronounce and have no idea what they are.
Be affordable: Like you I have tried diets that have a list of ingredients that break the food budget. Stick with common everyday foods that won't cost you an arm and a leg.
Items three and four are closely related to item two. Buy economical foods that are available in your local food stores and are common "comfort foods". I have a favorite health food store, but it's a good forty minutes away and every time we go we return with items not on our list. Usually it's some exotic food item we just wanted to try, usually it's pretty expensive and several time we have put it in the fridge or pantry and forgotten about it.
Items five and six are also similar. Of course the food we eat must be healthy or else what's the point! And if it is healthy we should maintain a healthy body weight. Again sticking to one or two ingredient whole foods and as much as possible organic foods is the way to go.
Let us look at items seven and eight.In order for this diet to work and be perfect it must be something that becomes a lifestyle and consists of foods that the whole family will eat. Now that's a tall order, especially if you have young children. Here are two helpful tips to get even finicky eaters to try new foods.
1. Let them help in the buying and preparing of the foods
2. Now and then it's okay to have the fries and burgers.
I believe that if you follow these eight guidelines you will almost automatically create a perfect diet.

Recovering From Total Joint Replacement


Immediately after your total joint replacement surgery, you will be moved into a recovery room to be monitored for a couple of hours. After the anaesthesia has worn off and you are awake, you will then be moved to your ward where you will stay for the next few days. A typical stay in the hospital will be between 3 to 5 days depending on your progress. Before the effects of the anaesthesia have fully worn off, you should consume pain killers to prevent a sudden onset of unbearable pain. So how do you recover from total joint replacement surgery?
Physical therapy
Muscle atrophy is the result of depriving muscles activities, leading to muscle wastage. This can happen after a couple of days post surgery, leading to shrinking of muscles and muscular apoptosis. In order to prevent this, physical therapy will be required. Physical therapy helps to strengthen the hip joints through various small but effective exercises. It usually begins almost immediately after surgery in order to prevent scar tissues from forming. A physical therapist will be there to teach you some basic exercises to help you regain strength and full hip movement in the shortest time. It continues even when you have been discharged, and you will be required to show up for your physical therapy classes up to 4 times a week.
Understand possible medical complications
Hip replacement surgery has a low risk and infections are at a low rate of 2%. However, this does not mean that you are completely clear of any complications. Blood clots in the leg veins can occur due to prolonged immobility, leading to serious consequences if they travel up to the heart or brain. Therefore, some surgeons will recommend the usage of compressive stockings.
Do not be complacent
As time passes, the hip will be stronger and the usage of walking aids such as crutches or walkers will no longer be required. Some patients will tend to get complacent and start to skip physical therapy sessions or ignore symptoms which may indicate infection. Therefore, always be mindful of your responsibilities as a patient and continue your physical therapy sessions until your therapist has given you the all clear.
Your new hip will be able to last you for at least a decade and provide you with freedom you never imagined. However, you still need to know that it is an artificial joint inside you and materials can fail due to various reasons and a second surgery may be required in future.

Treatment of Hallux Limitus


When a patient presents with symptoms of hallux limitus, it is important to take into account the severity of their deformity as well as the level of pain and how the pain limits daily activities in order to determine what treatment would be most appropriate. A variety of treatment options can be considered, including both conservative and surgical procedures.
Early stages or minimal discomfort from hallux limitus are often approached using conservative treatments. One popular method of conservative pain control is to take oral nonsteroidal anti-inflammatory drugs (NSAIDs), such as Advil or Motrin. Keep in mind that taking this medication does not cure the condition causing the pain. This method of treatment can be used to treat acute flair-ups of pain and swelling.
Foot orthotics are another conservative option to treat hallux limitus and have been shown to relieve pain better than the use of oral anti-inflamatories alone (1). Examples of custom modifications of orthotics used to treat this condition include the Kinetic Wedge and Morton's extensions. These specialized foot orthotics can help alleviate pain in the 1st MTPJ. By providing functional correction of deformities with the proper orthotic, 1st MTPJ discomfort can be improved. Foot orthotics are inserted into shoes and can be moved from shoe to shoe.
Physical therapy is also another conservative option that may be of some benefit in early stages of the condition.
Steroid injections can be administered in the doctor's office. A steroid or cortisone shot is given into and around the joint this may help to relieve pain. The reduction in swelling and pain from this type of injection may not last long. The injection can be repeated, however since over use of steroid can breakdown tissue there is a limit to the number of injects patients can receive.
If the conservative therapy that has been reviewed does not reduce pain, then surgical intervention may be necessary. The major goals of surgery are to reduce pain and improve daily function. Depending on the nature of the patient's foot and the degree of joint destruction, other goals could include improving joint motion, recreating a joint space in the 1st MTPJ and reducing deformities of bone.
Operations that are able to preserve the joint involve removing any abnormal bone formations around the joint in order to improve mobility. However, there are instances in later stages of hallux limitus and hallux rigidus where preservation of the joint is not possible. In this case, the joint articulations or cartilage may be removed and replaced by an artificial joint or the joint may require to be permanently locked together also known as joint fusion.
If you believe you are feeling symptoms of hallux limitus or hallux rigidus, it is important to address the pain with your doctor as early as possible. May people that have pain in this area assume that they have a "bunion". An earlier and accurate diagnosis opens the possibility of a variety of noninvasive, conservative treatments that can help reduce pain while walking.

Driving Health With Electricity Through Protein Flow In Ion Channels


With the knowledge of the human genome structure we are now entering the phase of learning how our body works using ion channels for protein placements within our cellular structures. The ion channels often are voltage related, or polarity related which allows the entrance and exit of multiple proteins. Prior to now we lacked the knowledge to see and understand the role of ion channels as well as we lacked the computing power to see the specific momentary actions of the ion channels. The ion channels are opening and closing in the tens of millions times per second, and we simply did not have the storage and speed necessary in computing power to analyze momentary events and summarize those processes for an overview of the sequences leading to health and disease.
For most lay people, such as myself, the concepts of ion channels and polarity can be difficult to understand. The explanations get muddled in the terminology used, such as this article itself referring to "voltage" and "polarity" at the same time. Let's clear up what is really going on and find a better way to understand electrotherapy and ion channels.
The structures in our body rely upon membranes. For our body the one very large membranous structure is our skin. Our skin serves to keep within and shield from out our internal organs while maintaining blood inside us. If we drive deeper into life we find a multitude of living structures that depend upon a similar "skin", or membrane. My husband's aorta split in the fall. It's winter now and he has been in the hospital for almost three months. He had two non-invasive surgeries, one major surgery, and four follow-up wound procedures. Now that his major wound is healing, according to federal regulations, he must be dismissed from the hospital. But he faces a huge barrier.
During life-saving surgery my husband had a spinal stroke and his legs don't work. Even with intense physical therapy, his legs will probably never work, and he can't return to our home because of all the stairs. Unless I can come up with a solution, my husband will be transferred to a nursing home. I decided to do a little detective work and it was an eye-opener. You may be in a similar situation. These criteria will help you choose the best place for your loved one.
Learn about your state's definitions and regulations. You may do this by entering the name of your state, followed by such words as statutes, definitions, licensure, fees, and inspection. If you're willing to use lots of ink and paper, print out this information for future use.
What rating does the nursing home receive from the state? My state, Minnesota, has categories of standards, including the minimum nursing staff, developing care plans, and quality of care. Four stars are the standard, yet many nursing homes in my area fail to meet it.
Meet with a social worker. The hospital my husband is in has social workers for each department. Two social workers contacted me immediately. I met with one of them and she was helpful and knowledgable about current regulations. A savvy social worker can save you time and angst.

Pick up brochures and other written information. Create a manilla file for each nursing home. Store these files in a handy place. After I visited the nursing home that had the wonderful brochure and a lesser reality, I jotted down notes about my visit on the brochure. "I'm not going there," my husband declared.
If you can, talk with families who have a loved one at the nursing home. What are the nursing home's pluses? What are the minuses? How does their loved one feel about the quality of care he or she is receiving? This is one of the most important decisions of your life. Make sure the nursing home meets personal and state standards before you enroll your loved one.
